چکیده

Recent advances in our knowledge of the neurophysiologic mechanisms underlying sleep and experimental states of coma (1) suggest the need for a neurophysiologic approach to the many complex clinical syndromes associated with disturbance of consciousness. The coma syndrome associated with hepatic disease and usually known as hepatic coma was chosen for investigation since it presents a good example of toxic coma without gross structural change in the brain. The rapid changes in level of consciousness which frequently occur in this syndrome make it particularly susceptible to experimental study. The observations described below represent an effort to analyze such electroencephalographic (EEG) patterns as may be observed in patients with various forms of hepatic disease before, during, and after the clinical state known as hepatic coma.

External ophthalmoplegia, alpha and spindle coma in imipramine overdose: case report and review of the literature.

A 13-year-old boy with imipramine overdose developed seizures, respiratory arrest, and coma. Abnormalities of oculovestibular reflexes, electroencephalograms, and brainstem auditory evoked potentials were monitored in relation to measurements of drug levels. An alpha-coma electroencephalographic pattern evolved into one evidencing spindle coma and eventually into a normal pattern. Prolonged bra...

Burst-suppression is reactive to photic stimulation in comatose children with acquired brain injury.

OBJECTIVE Burst-suppression is an electroencephalographic pattern observed during coma. In individuals without known brain pathologies undergoing deep general anesthesia, somatosensory stimulation transiently increases the occurrence of bursts. We investigated the reactivity of burst-suppression in children with acquired brain injury. METHODS Intensive care unit electroencephalographic monito...

Disorders of renal function in hepatic coma.

Renal failure is a frequent complication in advanced liver disease (Conn, 1973; Papper, 1978). Gastrointestinal bleeding and forced diuresis may be the causes in fulminant hepalic. failure and cirrhosis (Hecker and Sherlock, 1956; Shear et al., 1965). Hepatic coma or an associated metabolic abnormality may affect renal function or aggravate pre-existing renal abnormalities in severe liver disea...

Predicting outcome from subacute unresponsive wakefulness syndrome or vegetative state.

Predicting recovery of consciousness in patients who survive their coma but evolve to a vegetative state (recently coined unresponsive wakefulness syndrome) remains a challenge. Most previous prognostic studies have focused on the acute coma phase. A novel outcome scale (combining behavioural, aetiology, electroencephalographic, sleep electroencephalographic and somatosensory evoked potential d...

Experimental acute hepatic encephalopathy: comparison of the electroencephalographic changes in the liverless and in the eviscerated rat.

1. The present work was carried out to compare the electroencephalographic changes in liverless rats and eviscerated rats and to determine whether substances released from the intestine, in particular ammonia, play a major part in the mechanism of hepatic encephalopathy. 2. The animals were prepared according to a three-stage procedure: ligation of inferior vena cava; 3 weeks later, end-to-side...
